Everything you need to get started with FollowCare — your handbook, documents, and a clear step-by-step guide to your first supported session.
Starting with FollowCare is simple. Here's what to expect from your first contact to your first support session.
Reach out by phone, email, or our contact form. We'll have a free, no-obligation chat to understand your needs and confirm NDIS eligibility. Interpreter support is available.
We work with you to develop a personalised Support Plan that reflects your NDIS goals, preferences, and any specific needs — cultural, language, medical, or otherwise.
We'll walk you through the Service Agreement and Consent Form at your own pace. You can involve a family member, carer, or advocate. No pressure — questions are welcome.
We introduce you to your support worker — matched to your cultural preferences and language where possible. Your first session includes a detailed handover and orientation.
Regular check-ins and reviews ensure your supports are always working for you. You can adjust, change, or end services at any time — you are always in control.
These documents belong to you. Download them, read them at your own pace, and contact us if you have any questions.
